The UP Office of Admissions reminds all UPCAT 2026 takers to submit their certified true copy of grades for Grades 8, 9, 10, and 11 by 25 August 2025, Monday.
Reminders:
Each sheet of the SF-10/Form 137 must have a wet signature of a Grade 12 school official (included in the list of signatories in Form 2A), along with their printed name and designation.
Ensure that complete grades for Grades 8, 9, 10, and 11 are included in order.
Only submit Certified True Copies (CTCs), as original documents cannot be retrieved once submitted to the office.
Place the documents in a sealed envelope (any size or color) with the school official’s wet signature across the flap of the envelope.
